SERPA - Accountable Care Organization
What is SERPA-ACO?
- ACOs are groups of doctors and other health care providers who voluntarily work together with Medicare to give you high quality service and care at the right time in the right setting.
- Your doctor has agreed to participate in a Medicare Shared Savings ACO and to work closely with other doctors and health care providers in the ACO to coordinate care for Medicare fee for service beneficiaries, like yourself, who have traditional Medicare.
- The ACO may share in any savings that result from providing you with high quality and coordinated care.
The goal of the SERPA-ACO is to lower the cost, improve the quality, and increase the coordination of our patients' health care.
The ACO relies on the concept of the "Patient-Centered Medical Home."
Patients who regularly see the same doctors benefit from the doctors' knowledge of their health and medical history. That familiarity means the doctors know what tests and procedures are necessary for the individual and can monitor the patient's health through time.
The SERPA-ACO has partnered with Medicare to set up its ACO. Medicare is providing the start-up money so that SERPA can set up the tracking and reporting infrastructure necessary to show that its clinics are working to improve care and save money.
Why participate in an ACO?
We think it's the right thing to do. We are all interested in quality and saving money. If we save money for Medicare, we will share in the savings.
The ACO will benefit if we can demonstrate that we are improving on 33 different measures of quality, including patient satisfaction.

- Medication Reconciliation
- Fall Prevention Assessment
- Depression Screening
- Hiring a Care Coordinator that will be responsible for tracking patients' care from all of their medical providers. You may be contacted by the Care Coordinator to help you and your medical providers understand your health issues.
