Insurance & Payments
You will be asked to bring your insurance card to every visit at check in. This is for both our clinic and you to be aware of what is covered by your insurance and what costs you may be expected to pay.
Co-Payments: We ask that all co-payments be paid at the time of service. This arrangement is part of your contract with your insurance company.
Uninsured: Our goal is to provide care that is affordable. Payment is expected at time of service, unless prior arrangements are made with the billing department.
