Welcome to our practice and thank you for your interest in our
clinic. This website is designed to acquaint you with our
services. If you have further questions, please talk with any of our
employees or physicians or e-mail us (do not e-mail questions about
your medical care - please phone - e-mail is not secure). If you experience any problems at
the Clinic, please notify the administrator so we may resolve them.
Appointments
Office visits are by appointment only to help us keep your waiting time to a minimum.
It is our goal to follow our time schedule as closely as possible. However, if an emergency arises that would cause a delay, we will attempt to keep you informed of your doctor's schedule.
If you need to cancel an appointment, please call as much ahead of time as possible, preferably at least 24 hours. That way your time can be given to another patient.
Credit Policy
We will submit primary and one supplemental insurance claim for you
- subject to your having given us current insurance information
prior to services being provided. This will be filed at no charge to
you. Copayments must be paid at the time of service and we expect
timely payment of deductibles and non-covered services. All unpaid
balances over 30 days past due will be subject to late charges at 16% per annum.
Approximately 45 days after we submit your claim to your insurance
company any unpaid charges will appear in the "patient's
responsibility" space on your statement and payment must be received
before the next billing date. The financial responsibility for
service provided to you remains yours.
If you are unable to pay your charges when they come due, please
contact our Patient Accounts Manager to discuss a payment plan.
Payment may be made by cash, check, ATM card, VISA, MasterCard or Discover Card.
Insurance
We "Participate" with Medicare, Nebraska Medicaid, Workers'
Compensation and some other health plans/PPOs. Please show your
insurance card on each visit.
Night, Weekend and Holiday Care
The Clinic has devised an "on call" schedule, whereby one physician
or physician assistant takes responsibility for all emergencies
arising outside of normal office hours. After any emergency care is
rendered to you by the "on call" physician or physician assistant,
the responsibility for your ongoing care will revert to your own
personal physician or physician assistant.
If after-hours or weekend medical help is required, please contact
the person "on call" through the York General Hospital Emergency
Room at 362-6671.
Medication Refills
If you need a refill of medication you are currently taking, please
contact your pharmacist. If authorization to refill your medication
is needed, the pharmacist will contact your physician or physician
assistant. Call several days before needed, in case your doctor is
away.
Other Services
Allergy Shot Program: This program is designed to reduce waiting time
to our allergy patients.
Laboratory: The Clinic has an accredited full service laboratory, staffed
by certified laboratory technicians. Services are available by appointment.
Government regulations require a written physician order for any laboratory testing.
Drug Screen Collection: The Clinic has an certified drug screen
collection and testing program available to employers and patients. We perform non-DOT collections and testing and are certified to perform DOT collections.
Obstetric Program: The Clinic, in cooperation with the York General
Hospital, offers a complete obstetric/prenatal program.
Radiology/Ultrasound: The radiology/ultrasound department is state
certified and is staffed by Registered Radiologic Technologists and a
Registered Sonographer.
Reference Laboratory: We refer pap smear slides, cytology specimens,
moles, lesions and some laboratory tests to our reference laboratory,
Pathology Medical Services, P.C. and Nebraska Lab Linc, in Lincoln, Nebraska. These labs bill the patient directly for their services.
